*Participation with Medicaid programs prohibits charging for no-show appointment or cancellation. If you have a Medicaid plan and miss or late cancel more than three appointments, your therapy with me may be terminated.

Other Info

• Your insurance may cover mental health services. Please verify that your insurance is active. You can call your insurance company and ask what your mental health coverage is, what your deductible is, and what your co-pay is so you know what to expect.

• Cash, check (made out to Kalamazoo Counseling & Wellness), and all major credit cards are accepted for payment. Credit card payments are taken through a HIPAA compliant site.

• In order to respect the time we have set aside for you, we ask that you kindly give 24 hour notice if you will be unable to attend your appointment. The cancellation policy states that appointments must be cancelled with at least 24 hour notice to avoid a late fee of $50. Additionally, if you do not show for a scheduled appointment you will be charged a $50 fee.
